To make an appointment call (01332) 271212 or call in at either surgery.  If you wish to receive an APPOINTMENT REMINDER BY TEXT MESSAGE, please update your details using the "Your Clinical Record" link at the bottom of this page.

Cancelling your Appointment      If you are unable to attend an appointment, please use the link on this website or call us on 01332 271212 to cancel your appointment. 

Out of Hours     

If you require emergency treatment for a potentially life-threatening condition you should dial 999 and ask for the ambulance service.  The following conditions are considered potentially life- threatening: 
Chestpain
Loss of consciousness  Breathing difficulties   Severe   loss of  blood

For any other condition which you feel cannot wait until your GP Surgery reopens, you should contact the Out Of Hours Service on 08444 122239 . Your call will go to our local out-of-hours service, Derbyshire Health United.
Depending on your problem, you may go on to speak to a nurse or doctor.  Where possible you will be dealt with in a single call, but when the service is very busy someone may call you back.
You may be asked to attend an appointment at the out-of-hours service headquarters on Duffield Road.

APPLICATION FOR NEW REGISTRATION.  

Please note that your registration can take up to 10 days to process. 

FAILURE TO COMPLETE YOUR DOCUMENTS CORRECTLY, OR TO BRING THE CORRECT IDENTIFICATION TO THE PRACTICE WILL DELAY YOUR ACCEPTANCE ON TO OUR LIST.

IDENTIFICATION:

All patients wishing to register as a permanent or temporary resident at Lister House and Oakwood Medical Centre will need to provide identification to confirm who they are and where they live.

PLEASE READ THE DOCUMENT BELOW CAREFULLY.

FORMS TO COMPLETE:

You may print off the Registration Form and Medical Questionnaire below, fill them out and bring them with you on your first visit to the practice. Alternatively, call in and request a registration pack from one of our Reception Team.

Registration Form    This needs to be filled in correctly. 

When you register you will also be asked to fill out a medical questionnaire (see link below). This is because it can take a considerable time for us to receive your medical records.  You may wish to print and complete this, and bring it to the practice with your Registration Form.

Please supply us with your current medication request slip (from your previous doctor) if you have one.
