Appointments 

To make an appointment call (01332) 271212 or call in at either surgery.  If you wish to receive an APPOINTMENT REMINDER BY TEXT MESSAGE, please update your details using the "Your Clinical Record" link at the bottom of this page.

Cancelling your Appointment      If you are unable to attend an appointment, please use the link on this website or call us on 01332 271212 to cancel your appointment. 

Out of Hours     

If you require emergency treatment for a potentially life-threatening condition you should dial 999 and ask for the ambulance service.  The following conditions are considered potentially life- threatening: 
Chestpain
Loss of consciousness  Breathing difficulties   Severe   loss of  blood

For any other condition which you feel cannot wait until your GP Surgery reopens, you should contact the Out Of Hours Service on 08444 122239 . Your call will go to our local out-of-hours service, Derbyshire Health United.
Depending on your problem, you may go on to speak to a nurse or doctor.  Where possible you will be dealt with in a single call, but when the service is very busy someone may call you back.
You may be asked to attend an appointment at the out-of-hours service headquarters on Duffield Road.

Please contact us as soon as you book your holiday, as to give protection against some diseases requires a course of vaccinations given over a few weeks.

Please complete one of our travel questionnaires (available by clicking on the Travel questionnaire link below).

You should then contact the surgery a week later to find out what vaccinations/anti-malarial medications are advised, and to book your appointment with the Practice Nurse if needed.

Please note that not all travel vaccinations are free. 

Travel Questionnaire

If you need to make an appointment with the practice nurse to discuss your travel arrangements, this will include discussions about which countries and areas within countries you are visiting to determine what vaccinations/ anti-malarial medication are required.  There is further information about countries and vaccinations required on the links below:
